They are often the first to knock on the door, the ones who listen without judgment and the steady hands that guide patients through some of life’s most difficult challenges.

Community Health Workers (CHWs) at Alameda Health System (AHS) don’t just work in the community; they are from the community. With deep understanding, shared lived experiences, and unwavering dedication, CHWs serve as lifelines for patients who might otherwise go unseen and unheard.

CHWs play a crucial role in addressing health disparities and improving access to care for our most vulnerable communities. They help reduce health inequities by promoting health literacy, increasing access to services and fostering trust between patients and AHS.

Whether helping someone find housing, navigate a medical appointment, or access food and transportation, CHWs remove barriers and build bridges to better health.

Their impact goes far beyond any single moment. By showing up with compassion and consistency, they ensure no one is denied the dignity and quality care they deserve.

This National Community Health Worker Awareness Week, August 25 – 29, we highlight the voices of our CHWs as they share what led them to this meaningful work and the patient moments that continue to inspire them.
