Care Transitions
I began my career at Alameda Health System (AHS) nine years ago in emergency department (ED) registration and admitting. Then three years ago, I made the move to become a community health worker (CHW) on the community health outreach transitions team. I made this move to help patients navigate the health care system by providing education, resources and support.
One patient I’ll never forget had been bedbound for 10 years and had not seen a doctor or even a family member in over five years.
After earning their trust, I became the first person to visit them in person. When they developed a life-threatening infection, they called me for help.
I stayed by their side through hospitalization, arranged permanent housing to meet their medical needs, and encouraged them to set boundaries and use their voice.
Becoming a CHW has been one of the most rewarding experiences of my life. It’s about compassion, advocacy and walking alongside someone until they’re strong enough to walk on their own.
