AHS Winning with 5-Stars!

Alameda Health System (AHS) announced that its skilled nursing facilities have earned the highest ranking of five-stars from the Centers for Medicare and Medicaid Services (CMS). According to CMS, only the top 10 percent of all nursing facilities evaluated in the state receive the five-star rating.
"The teams are very dedicated and focused on resident centered care. They've collaborated together to ensure our residents are receiving the highest level of care possible. AHS' four SNF locations (Park Bridge Rehabilitation and Wellness Center, Sub Acute, South Shore and Fairmont Hospital) all receiving CMS 5-Star ratings is a direct reflection of the care and commitment our staff embody."
~Richard Espinoza, Director of Long Term Care Operations, Alameda Hospital
The CMS publishes the rankings of all Medicare- and Medicaid-certified skilled nursing facilities on its Nursing Home Compare website, which allows consumers, families and caregivers to compare facilities and decide which nursing home will most fit their needs.
Star ratings are based on the skilled nursing facility’s health inspections from the last three years, staffing levels, and quality measures. The individual scores are combined to determine the overall rating.
Quality measures at the skilled nursing facilities were reduced to below (lower percentages are better) state and federal measurements in the following areas:
Patient falls causing injury
Pressure ulcers in short-stay residents
Urinary tract infections
Unhealthy weight loss
Symptoms of depression
Use of anti-psychotic medications in long-stay residents
None of this would have been achieved without our dedicated and hard-working staff. We extend a big thank you to the teams of each location who focused on improving our quality measures/indicators in order to provide the highest level of care possible to our residents.
